Foster said the center’s model embeds withdrawal management into crisis services to address the frequent overlap between mental illness and substance use and to provide “no-wrong-door” continuity of care. She said the unit will connect patients to inpatient treatment providers, outpatient partners and peer supports when appropriate, and will integrate work by the center’s sobering unit and EMS partners for initial medical monitoring.

Staff described partnerships already in place, including inpatient referral coordination with ASAP, medication support from Care Pharmacy, ambulance support and coordination with local peer-recovery organizations. Foster said the county used opioid settlement funds to hire a medical director and cover startup expenses.

County staff flagged several challenges: Iowa’s licensing process is backlogged, workforce shortages (including a current nursing vacancy), and uncertain long-term payment arrangements. Foster said two managed-care organizations had expressed willingness to contract for the service but a third had been reluctant; she noted that private insurance often does not cover crisis-based services. Foster offered local utilization data to show demand: she said UnityPoint Cedar Rapids logged 800 people seeking withdrawal-management services and that 11 withdrawal-management licenses existed in Iowa, with less than half accepting Medicaid. “One is the one community-based withdrawal-management program that will open in the state and that will be us,” Foster said.

Next steps include obtaining a temporary six-month license, completing a state site visit during that period and then pursuing a full license.
