McKinnon said the adopted HSH portion of the OCO fund totals $463,000,000 for FY 2025–26 and $320,000,000 for FY 2026–27, and that the package includes $104,000,000 in new programming over the two-year window.

The presentation enumerated major new investments added during Board phase: roughly $46.9 million for new rapid-rehousing and shallow subsidy programs across populations (including 130 family rapid-rehousing slots and allocations to support family and adult rapid rehousing), $31.8 million to fund up to 120 beds for the Rapid Engagement Shelter and Treatment for Opioid Recovery (RESTORE) program, $3.4 million for 50 new adult hotel vouchers tied to the mayor’s Large Vehicle Strategy, and $4.7 million for outreach, case management and a vehicle buyback program to support individuals living in vehicles. Committee members focused most of their questioning on whether certain adult rapid-rehousing slots were being set aside to support the RV (vehicle-living) initiative and how that would affect prioritization through the city’s coordinated entry process. Member Friedenbach pressed staff for clarity, saying, “If you’re homeless, living in an RV, you’re homeless. If you’re living on the street, you’re homeless,” and asked whether people in RVs would receive preference over unsheltered people without vehicles.

McKinnon and HSH staff responded that the 100 adult rapid-rehousing slots identified in the budget were intended to add capacity to support the RV intervention strategy — not to remove slots from the baseline coordinated entry system — and that she could not answer how coordinated entry prioritization rules would be applied at the case-management level. “I can’t speak to the coordinated entry process. Several members asked about the legal review of those Board actions and whether the city attorney had provided an opinion. The assessment reported about 7,000 households counted during the 2024 PIT; when annualized to reflect inflow and service use throughout the year, the report estimated roughly 20,000 households experienced homelessness in 2024.

Key findings the controller’s staff flagged: the PIT showed a roughly 7% increase in homelessness since 2022 driven largely by an increase in sheltered homelessness; family homelessness increased substantially (about 82%), a change the authors said was partly attributable to a methodology change that identified more families; and the inventory of emergency shelter and transitional housing on the night of the PIT covered about half of people in need (roughly 4,000 beds available and 4,000 people in shelter/transitional housing that night). The assessment also drew on DPH data estimating that more than 16,000 unhoused individuals had a serious mental health and/or substance use diagnosis in 2024.

The draft assessment identified that single adults — who represent the majority of people counted — have higher shares of acute-care needs (serious mental illness, substance use disorder, developmental disability and chronic homelessness) and higher shares of physical health and disability needs. Families, while smaller in count, were more likely to be younger and to report fleeing domestic violence and to need confidential and family-sized housing options; the assessment also noted that families face specific barriers such as lack of documentation, larger unit size needs and income disqualification for some subsidies.

The assessment concluded that despite increased shelter and housing capacity, unsheltered homelessness was largely unchanged and that the system is seeing a high rate of inflow. It recommended aligning inventory with the changing needs of populations: more prevention and housing-stabilization services for people at risk; interim housing with supports or permanent supportive housing for single adults with acute needs; and targeted short-term financial and rapid-rehousing supports for families.

Committee members asked for more detailed data and follow-up. DPH described several capital and operating investments funded in part with local Proposition C dollars and state grants: funding to open roughly 180 new residential care and treatment beds (including the Eleanor Fagan Center, also called the Keane Hotel, and Wells Place — the Marina Inn operated by the Salvation Army), operating funding included for 44 residential care facility beds at a purchased site at 601 Laguna Street, and ongoing work on the Treasure Island Behavioral Health Building (a new ground-up facility expected to open in 2028). DPH also reported purchasing 1660 Mission for a Health Recovery and Connection Center that will co-locate outpatient behavioral health, buprenorphine treatment access, City Clinic services and drop-in supports; the 1660 Mission item was pending a full board vote at the time of the committee meeting.

Radhika said DPH has drawn down significant state capital awards and other one-time funding since 2022. “We were awarded almost $30,000,000 in Prop 1 Bond B CHIP funding...and we have been awarded over $88,000,000 in state capital funding for behavioral health projects since 2022,” she said.

DPH staff cautioned the committee that the two-year plan relies on one-time fund balance and acquisitions; the department projected a “$40,000,000 one-time cliff in fiscal year 'twenty seven-'twenty 8” for operating funding if new revenue projections or reconciled underspend are not realized.
