Pueblo County approves two-month extension of Resource Exchange subcontract for Medicaid case management

PUEBLO — The Pueblo County Board of County Commissioners voted April 29 to approve a two-month extension of the county's subcontract with The Resource Exchange, which provides case management for Medicaid home- and community-based long-term care services through the Pueblo Single Entry Point program.

Rejean Ricciton, adult programs administrator for the Pueblo County Department of Human Services, told commissioners the county has subcontracted with The Resource Exchange since March 2024 and requested the extension because the current subcontract ends April 30, 2025. Ricciton said negotiations produced a revised statement of work to meet "the health care policy and financing contract requirement" that establishes a case manager-to-client standard of 1 to 65; Pueblo County is currently operating with caseloads "close to the nineties." The new subcontract and the revised staffing plan are scheduled to take effect July 1, 2025.

"This will be a phased approach to allow TRE to hire more staff to accommodate the cases we will be transferring," Ricciton said, explaining the extension is meant to avoid service interruption while the county and subcontractor finalize terms and staffing adjustments.

Commissioners asked clarifying questions limited to the requested extension. The board chair asked for a motion to approve item 2A; a commissioner moved and another seconded. The record shows all commissioners present voted in favor and the motion carried. No roll-call vote with individual yes/no names was recorded in the transcript; the board approved the resolution extending the independent contractor agreement by two months.

The extension preserves continuity of case management services under the Single Entry Point program while county staff and The Resource Exchange finalize a contract that aligns with statewide program changes and the county's plan to meet a 1:65 caseload ratio.
