Bluegrass Care Navigators outlines PACE and community navigation partnership with FCPS

Board members heard a presentation from Bluegrass Care Navigators about community resource navigation, senior services and a partnership with the district Learning Center.
Miranda Scully introduced Jerry Bodd Stewart, identified in the meeting as the PACE outreach coordinator with Bluegrass Care Navigators. Stewart described how her partnership with Fayette County Public Schools began long before her current role and said Bluegrass Care Navigators provides workshops, one-on-one resource navigation, grief support, mental-health awareness and help with advanced-care planning. "We deliver support to the students, families, and staff through workshops... and we try to go in and bridge any gaps that are there," Stewart said.
She described PACE (Program of All-Inclusive Care for the Elderly) services for people aged 55 and older, dementia care, palliative care, hospice, grief services and advanced care planning supports. Stewart also described local community partnerships including the community council, Parent Advisory Council and FACE team that coordinate resources for families.
Board members thanked the presenters and noted the district values partnerships that extend services to students, families and staff. No board action was taken; the presentation was accepted as information.
