Kenosha board to review Head Start corrective action plan after unsupervised-handling incident at Chavez

Kenosha School District administrators presented a corrective action plan Thursday addressing a Head Start noncompliance finding after an incident at Chavez School in which staff carried a child into a chair in a manner determined to be inappropriate.
Rhonda and Michelle (Head Start staff) explained the district self-reported the incident to administration and the regional Head Start office. The child was not injured and the parent did not file a formal complaint, the presenters said. The regional office investigated and required a corrective action plan, which district staff and the Head Start leadership team developed and now seek the board’s approval.
The corrective action plan lays out specific measures, including immediate retraining on safety procedures, documented supervision plans for teaching teams, social-emotional instruction to reduce behaviors that may escalate, additional communication protocols, monthly check-ins with Head Start staff and documented monitoring of safety procedures for new hires. The plan also calls for providing staff with radios (walkie-talkies) to summon help during behavioral incidents.
Attachment A to the board packet summarized the Head Start performance report describing the incident; Attachment B is the 120-day corrective-action schedule district staff proposed; Attachment C includes Head Start health-and-safety guidelines; and Attachment D provides an “active supervision at a glance” checklist the regional office uses.
Rhonda said the plan and its attachments must be approved by the KUSD board and the district must present evidence of completed actions to the Head Start regional office by June 6. Board members asked clarifying questions; one correction of a name in Attachment A was noted and fixed.
The board did not record a final vote at the organizational meeting; administrators said they will return with documentation showing completion of the corrective actions for submission to the regional Head Start office.
