Residents raise ER cleanliness, staffing and service concerns; commissioners send letter of no confidence to Hidalgo Medical Services

Multiple members of the public used the board’s public-comment period July 10 to describe alarming conditions in the Hila Regional Medical Center (GRMC) emergency department and to urge county action.
William Lloyd, a downtown business owner, described repeated problems with his wife’s emergency visits, saying staff treated her dismissively and that he documented “puddles of yellow liquid on the floor” and extensive grime at the junction between floor and wall. “This emergency room is a health hazard. It should be shut down today until it is clean,” Lloyd said. Other commenters also recounted poor personal experiences and urged the commissioners to press the hospital and other providers for improvements.
Hospital Chief Executive Officer Robert Whitaker addressed the board in a later presentation and reported that May and year-to-date volumes were higher than the prior year across admissions, emergency visits and outpatient services. Whitaker said the hospital recorded an operating surplus of about $565,000 for May and a year-to-date operating surplus near $4.3 million, excluding certain program dollars, and that the board had approved operating and capital budgets for fiscal year 2026. He said capital spending would include a planned CT scanner replacement, with state capital dollars allocated for that purchase. Whitaker also announced physician recruitment successes: internal medicine physician Dr. Teresa Arazaga was slated to join the GRMC medical group the following week, and the hospital had signed a contract with a urologist expected to start in the fall pending licensing.
Separately, the board addressed the county’s contractual relationship with Hidalgo Medical Services (HMS), which operates senior centers and other services under county agreements. The county manager read a draft “letter of no confidence” that said HMS had “lost the trust of citizens of Grant County” and had not maintained staffing to provide promised services at Tucasa and senior centers. The letter said HMS had provided notice of termination of senior-center agreements after county-requested audits of HMS records and that HMS leadership had blamed funding shortfalls while not fully documenting the use of funds.
Commissioners moved and seconded the letter of no confidence and approved it after brief discussion. Commissioner Stevens said he had attended a follow-up community meeting with HMS leadership and had hoped to hear stronger acknowledgement of problems and concrete steps to fix them; he said those acknowledgements were not forthcoming. The board instructed staff to send the letter to HMS’s board of directors and requested that HMS address transparency and leadership issues to restore community trust.
The meeting did not adopt new county appropriations for hospital or HMS services at the time. Commissioners and meeting speakers requested follow-up: more community town halls by the hospital, monitoring implementation of advertised staffing and service improvements, and documentation from HMS showing use of county funds and an action plan to restore service delivery.
No formal enforcement action against GRMC was taken at the meeting. The county’s letter to HMS is an official statement of the board’s concerns and will be transmitted to the HMS board of directors for response; the county indicated the matter could return to a future agenda pending HMS’s response and any required corrective actions.
