Strive CCS adds five providers, consolidates lead role and will share contact list

Program staff reported five new provider contracts that will serve both Vernon and Crawford counties, announced a consolidation of the lead role to one salaried staff member, and said vendor/provider contact information will be shared with service facilitators and external vendors.
Sarah, a staff member, told the Strive CCS advisory group that five new provider contracts have been signed and the new providers are largely skills-development and wellness vendors who will serve both Vernon County and Crawford County. “We have 5 new providers that contracts have been signed and they've been added to our provider list,” Sarah said.
The provider network discussion focused on coverage and information-sharing. Sarah said she asks prospective providers whether they can cover both counties and that, while larger partners may be more localized, the recent contractors have agreed to travel as needed. “All of the providers that, are recently contract contracted are willing to travel to both Crawford County, and Vernon County and cover all areas,” Sarah said.
Committee members discussed giving vendors access to a consolidated contact list. Sarah said Honey also maintains an updated provider list with names and email addresses; Tony said Dawn will manage and distribute a service-coordinator contact sheet. Tony described an internal reorganization intended to improve consistency: “We consolidated the lead role into Dawn, and we've actually made her a salaried employee. And she'll be kinda taking lead with mentoring and everything like that.”
Members raised questions about how independent contractors can continue serving clients after discharge if services must be billed through Medicaid (ForwardHealth) or as private pay. Sarah said the committee can ask providers directly whether they are certified under a card service and have an MPI number with ForwardHealth. “We usually could just go back to the provider and asks them, you know, if they're certified under a card service under an MPI number for forward health,” she said.
The committee scheduled a vendor meeting presentation by Northwest Connections’ regional mental health lead, Katie Herb, who will present virtually at the upcoming vendor meeting on June 27 to explain how Northwest Connections operates in the region and to answer vendor questions about county differences.
The group also confirmed that the contact sheet and provider list will be distributed to internal staff and independent vendors to improve referral and continuity of care.
