Summary
The Greendale Health Department announced a two-part 2025 Community Health Assessment: a 17-question community health survey and a PhotoVoice initiative. The surveys open through March 16, with QR codes and outreach planned at library and community events.
The Greendale Health Department on Feb. 18 described plans for the 2025 Community Health Assessment, centered on two linked public-engagement efforts: a short community health survey and a PhotoVoice (Lens on Reality) initiative.
Survey and PhotoVoice details
Amanda, the health-department spring intern from UW–La Crosse, said the community-health survey — “Your Voice, Your Health, Community Well-Being” — has 17 questions and takes roughly 5–10 minutes to complete. Responses will be analyzed alongside county and CDC quantitative data to identify local health trends.
The PhotoVoice project invites residents to submit photographs that represent how they experience Greendale, with a short narrative to explain the selection. PhotoVoice entries can highlight positive, negative or neutral aspects of community life; a social-media nomination challenge is optional for participants.
Access and outreach
Paper and online survey copies will be available; the department will distribute flyers and place QR codes on social media and around town. Amanda began outreach immediately (senior social club, library events) and said paper copies can be requested from the health department. The survey period is open through March 16; QR codes were expected to be active the day after the meeting.
Purpose and next steps
Staff said the combined qualitative (PhotoVoice) and quantitative survey approach will guide the health department’s priorities and programming for the next five years. Results will be used to plan public-health activities, outreach and resource alignment.
How to participate
Residents who live, work, play, pray, or study in Greendale were invited to take the survey and submit PhotoVoice entries. Those without smartphones may contact the health department general email to receive questions and submit responses.
