State survey finds multiple deficiencies at county nursing center; one G-level pain-management citation may draw fine

Stephenson County Nursing Center administrators reported the facility’s recent state health survey produced multiple tags, most classified as low scope and severity, and one G-level violation for pain management that could result in a fine.

Administrator Marcos presented the survey findings item-by-item: F602 (free from misappropriation/exploitation) and related tags concerning missing medications; F604 (restraints) cited use of bed bolsters now considered restraints; F607 and F609 (abuse policies and reporting) tied to a nurse who failed to follow reporting procedure; several quality-of-care and dietary tags (including F684, F686, F803 and F812) and training/documentation concerns for agency staff. The only higher-severity citation was F697 (pain management), a G-level finding tied to a delay in a medication ordered for a resident who was dying; Marcos said the facility may face a state fine for that citation.

Life-safety survey items included four electrical and egress issues: a door in the Alzheimer’s/dementia area that does not push out as expected, a missing emergency exit sign in the Oakwood area, improperly used power strips in an office and a sprinkler obstruction related to recent HVAC work. Marcos said staff will submit plans of correction, perform required education, and expect either a desk review or a revisit—because of the G-level tag, surveyors are likely to return for an on-site revisit and will review the facility’s corrective documentation.

Administrators told the committee they will compile a binder of corrective action, training and documentation for the revisit. For most tags the facility will submit documentation through a desk review; the pain-management citation will require more intensive follow-up.
