Commissioners amend Core Physicians contract, table PointClickCare subscription; approve retroactive Silverthorn day program payment

Rockingham County commissioners on Feb. 13 approved an amendment to the county’s medical director and physician-services agreement with Core Physicians LLC that will change provider payment terms and take effect March 24, 2025. Commissioners also voted to table a proposed PointClickCare Meal Time Solutions subscription while staff completes a review of a dietary services RFP, and approved a retroactive adult medical day care payment to Silverthorn to cover co-payments while Medicaid eligibility processed.
The senior director of long-term care services explained the Core Physicians amendment reconciles prior coverage after one of the practice’s doctors left. The amendment allows four remaining providers to split certain compensation back to them (each receiving 25% of the redistributed amount) and incorporates a 5% rate increase that will take effect in April. The director said some names were removed from the provider list, a nurse-practitioner clause was removed, and contract language was adjusted to stop paying for consultant hours that were not being used.
On the PointClickCare Meal Time Solutions subscription, staff said the software is intended to replace the county’s current nutrition-management system, but several vendor proposals in the ongoing dietary RFP referenced different platforms. Staff recommended tabling the subscription because the contract would not start until March and the county may select a vendor in the RFP that uses a different nutrition platform. Commissioners voted to table the subscription so staff can finish vendor comparisons and confirm whether the county should adopt the platform being proposed now.
Commissioners also approved a request from the county’s long-term care program to cover the co-pay difference for a Silverthorn adult medical day-care client (identified as “LV” in the record) at $15 per day, up to five days per week, retroactive to Nov. 1, 2024. Staff said Medicaid processing delays can create retroactive billing situations; the county will cover the difference that otherwise would have been paid if Medicaid had been in place earlier.
Votes: the Core Physicians amendment passed on the record; the PointClickCare subscription was tabled; the Silverthorn payment was approved. Staff said they will proceed with implementation steps for the Core Physicians amendment and will continue RFP evaluations for dietary services.
