Board reviews NASF form and regional tabletop exercise; staff to circulate contact forms

Health department staff told the Board that the Name, Address, Phone, Health/history form (NASF) — used to speed prophylaxis or medication distribution to board members and responders in emergencies — should be updated annually and returned to the department.
The director explained that the NASF dates to post‑9/11 prophylaxis planning and lets the department know household composition and medication needs so board volunteers can be served promptly at closed dispensing points: “This form lets us know because the board are first responders in a case of emergency… we take care of our board first.” Staff said they will email a PDF of the NASF and accept returns by email or at the next meeting.
Staff also described a regional functional exercise (tabletop) scheduled for Feb. 4 that will focus on FEP (functional needs, communications and partner coordination) and that primary participants are invited agencies: health departments, EMA, fire, police, hospitals and access & functional needs partners. The exercise runs 8 a.m. registration, 8:30 a.m. start and a “hot wash” to 12:30 p.m.; after‑action reporting will follow and the board will receive the subsequent after‑action report and improvement plan.
Board members were told the exercise is invitation‑only and intended for partners and response agencies rather than the general public. Staff invited board members to register if they wished to observe or participate and noted a summary report will be provided to the board afterwards.
