Bernalillo County, Albuquerque to host Unite Us summit to streamline referrals for people experiencing housing and food insecurity

Bernalillo County and the City of Albuquerque will host a summit for community service providers on Feb. 6 to introduce and onboard agencies to Unite Us, a closed-loop referral platform designed to coordinate referrals for food, housing and behavioral-health services.
The event will run from 11 a.m. to 3 p.m. at Los Duranes Community Center in Albuquerque, organizers said. It will include panel discussions with provider agencies, a question-and-answer session, lunch and opportunities to schedule onboarding with a Unite Us representative. Bernalillo County staff said the platform is free for nonprofit providers to join.
"It's a closed loop referral system where agencies can connect clients through the web platform without having to send paper referrals, phone calls and get instant outcomes," said Sarah Spain, Bernalillo County transition planning services manager. Spain said the platform allows agencies to complete screening tools that capture social determinants of health and to view a client's social-service history so the client does not have to repeat their story.
Host Esteban Vasquez, speaking on the Bernco Beat segment announcing the summit, said the goal is to build a stronger network among healthcare providers, nonprofits and other partners so referrals result in actual connections back to the referring agency.
Spain said agencies already using the platform in Albuquerque and Santa Fe County will speak on a panel about their experience, and Unite Us staff will be available to schedule onboarding for agencies ready to join. She emphasized that the system closes the loop by returning outcomes to the referring agency when a client connects with services.
Organizers encouraged providers to register in advance. For more information, Bernalillo County staff directed listeners to bernco.gov and to search for "Bernco Unite Us Summit."
