Physician presents Medicare GUIDE pilot offering respite and care navigation for local veterans with dementia

In his presentation, Dr. Eric described five components of the model as he and his team implement them: respite care, care coordination, in-home response for urgent concerns to avoid unnecessary hospital trips, data reconciliation across care settings, and caregiver support. He said eligibility requires a dementia diagnosis and straight Medicare (patients enrolled in hospice, PACE or other non-traditional Medicare arrangements are not eligible, per his explanation).
On respite, Dr. Eric said Medicare under the GUIDE model will cover a period of in-home respite care and offered an estimate during the meeting of "up to 85 hours" of respite support and an additional financial cap he discussed as approximately $2,500 in examples offered to illustrate potential benefit limits; those figures were presented by the speaker and should be verified with Medicare guidance and local providers before relying on them for eligibility determinations.
Alan Majeski of Pape Home Care introduced the presentation and said his agency plans to help educate veterans in Miami Lakes about benefits they may not know exist. Majeski said his organization intends to coordinate outreach with veterans groups and the American Legion to connect eligible veterans with providers participating in the GUIDE model.
No committee action or formal referral followed the presentation; members thanked the presenters and asked staff to share contact details for follow-up outreach. Committee members suggested the presenters consider outreach at American Legion meetings and other veteran-focused events to reach the targeted population.
The committee did not adopt any policies or make formal determinations about local funding related to the GUIDE model during the meeting.
