Items included medication-administration timing errors that exceeded the allowed tolerance (some doses were approximately 20 minutes late); the facility said it adjusted certain medication schedules (for example, moving some medications to 8:30 a.m. or to 5:30 a.m. for those needing pre-meal dosing) and that follow-up audits showed improvement. The facility also received a citation for leaving some medications at bedside for residents unable to self-administer; staff completed reassessments.
Surveyors noted missing background-check documentation for hires in one year (2023); administrators said they located background checks for other years and have placed records in a centralized binder. Food-service and sanitation citations included undated opened food boxes and several days of dishwasher temperature records below required thresholds; staff said they corrected procedures and documentation. Infection-prevention observations included one LPN observed during wound care who omitted some hand-hygiene steps; the facility reported retraining.
Life-safety items included incomplete emergency-operation binders (staff created binders and updated the emergency plan), missing semiannual sprinkler-system testing (the facility contracted a vendor to perform semiannual tests), emergency-light battery tests that were not on the required schedule (corrected with a weekly test log and an annual 90-minute load test), and use of non–medical-grade electrical receptacles in clinical areas (the facility ordered and installed green-dot medical receptacles).
Helm described the survey as an opportunity to improve policies and procedures and said most findings were corrected through plan-of-correction steps; the facility indicated it is now in compliance with the cited items.
