Staff displayed the current form and one or more draft front-and-back versions of the proposed new form. Staff said the primary change since the last discussion was a minor wording adjustment under a “please note” section to specify a box where two commission members are identified for review.
A commissioner moved to accept the redesigned COA application form; the motion was seconded and the commission voted in favor with all commissioners present saying “aye.” The commission recorded the motion as approved.
Staff said, if approved, the commission intends to implement the revised form in practice and continue to refine materials as needed. No additional conditions or revisions were recorded at the meeting.
