County adult community justice seeks health‑related social needs capacity building grant to expand housing supports

Deschutes County’s adult Community Justice division received board approval May 20 to apply for an Oregon Health Authority‑funded health‑related social needs capacity-building grant administered through PacificSource. The application seeks capacity‑building funds to allow the division to bill Medicaid‑aligned HRSN funds for housing supports for clients on supervision.
Trevor Stevens, business manager for Community Justice, said the HRSN program can fund up to six months of services through coordinated health plan billing, and that people released from incarceration are a priority population. The county does not currently bill OHP for adult corrections cases and needs administrative capacity — data systems and billing knowledge — to do so. The proposed award would provide up to $353,000 over an 18‑month period to build that capacity.
Commissioners approved the application unanimously and asked staff to coordinate with existing local HRSN providers (First Place, Changing Patterns and others) to leverage partner case management. Staff noted the award schedule, if selected, would likely start in late 2025 after award and fund release timing, and the county will continue pursuing HRSN funding capacity even if not selected for this round.
