Deputy Director Caitlin Acevedo told the San Francisco Entertainment Commission on April 1 that staff issued three notices of violation in March after inspectors observed entertainment or sound conditions that violated permit limits.
Acevedo said inspector Adams responded to two separate sound complaints at Reverb, 2801 Leavenworth Street, on March 16. Adams parked across the street, heard bass while the doors were closed and observed a DJ performing. Measurements showed Reverb operating about 7 decibels above its approved dB limit, and the venue was issued a notice of violation on March 19 for operating without a final permit in place. Acevedo said Reverb has since completed the final DBI inspection and now holds POE and EHP permits; no complaints were received after that weekend.
The commission also heard that Lion’s Den, 57 Wentworth Street, received a March 19 notice of violation after an inspector’s March 16 visit found the venue exceeding its allowed sound level, doors open during entertainment and no on-site staff familiar with permit conditions. Acevedo said the inspector measured sound at the sound board (the permit’s designated spot‑check location), brought levels down into compliance, and the operator Steven Lee held a staff meeting to correct the behavior.
A third notice was issued to the Blue Light, 1979 Union Street, after an inspector visited on March 22 and found a DJ performing after the venue’s approved LLP hours with doors open. Acevedo said sound measurements complied with the approved limits, but the entertainment time exceeded the permit’s allowed hours; the owner, Johnny Matheny, told staff he would address the issue with employees.
Acevedo described the violations as enforcement actions the office will follow up on; she answered commissioners’ procedural questions about complaint reporting and explained inspectors set official indoor and outdoor sound limits during on‑site testing. Commissioners and staff emphasized the role of outreach and a hotline in resolving neighborhood concerns in real time.
The commission did not take any formal vote on enforcement actions during the meeting; the report summarized inspections and enforcement steps taken by staff.
