Robin Coos and county procurement staff described a multi‑month RFP process that produced six initial proposals; the county interviewed four respondents and later evaluated two finalists. The expanded staffing model used for apples‑to‑apples pricing included two nurse practitioners, two LPNs and an administrative position. Under that model ProactiveMD—the incumbent—submitted a renewal price materially higher than the current annual cost; staff summarized the renewal as approximately $1.2 million annually versus current annual cost near $784,000, driven largely by the increased provider staffing.
Presenters said the expanded model was intended to reduce the clinic’s wait list and to allow more county employees and dependents to be served; county staff noted current enrollment/usage is about 37% of eligible employees (roughly 800 users of about 2,000 eligible employees). Commissioners expressed concern about continuity of care if the vendor changed, staffing timelines, and whether an on‑site medical doctor (MD) should be part of the clinic model. Several commissioners requested additional information before finalizing a multi‑year renewal: (1) one‑year pricing for ProactiveMD, (2) the contract’s early‑termination penalty, and (3) the incremental cost to include an on‑site MD rather than an MD in oversight/telehealth.
After discussion the board approved placing the renewal on the April 21 consent agenda and directed staff to get the one‑year pricing, the termination penalty amount, and an estimate for an MD on site; commissioners noted that hiring clinicians can take several months and that staff should plan implementation timelines accordingly.
