Council asks staff to add HMIS data-flow and measurable homeless-action objectives

Council members said the city needs a by‑individual (not aggregate) view of service flows so the city can track unique people through outreach, shelter, transitional and permanent placements and avoid double counting across provider reports. Council asked staff to define the HMIS data flow, describe what local partners (including the CRC and private nonprofits) contribute, and provide a staff objective to develop a data-architecture diagram and reporting approach that preserves confidentiality while allowing outcome tracking.
Members also asked staff to move from “continue” language to concrete objectives: for example, identify service gaps, specify goals for mandatory recovery pathways where authorized by law (a reference to post‑Prop 36 legal changes was raised), and set measurable targets for outreach teams such as the HOPE team. Staff indicated these are staff-level objectives and agreed to produce a proposed objective for inclusion in the next draft.
Council directed staff to bring the HMIS/data-flow objective back with timelines and metrics as part of the April 9 follow-up.
