Mayor’s budget instructions shape SFHSS planning as new mayor announces hiring restrictions

SFHSS Chief Financial Officer Iftikhar Hussain presented the mayor’s budget instructions and the department’s schedule for budget development on Jan. 8, telling the Health Service Board that the department will incorporate the mayor’s directive to pursue vacancy reviews and efficiency measures as the city confronts a projected multi‑year general‑fund shortfall.

Hussain described the SFHSS budget footprint: benefit trust activity is the largest fiscal impact, while the department’s general fund administrative budget is relatively small. He said the mayor’s instructions focus departments on core activities and targeted reductions to address a projected cumulative citywide budget deficit. He described a timeline for SFHSS budget briefings, including an upcoming Finance Committee meeting and the board’s role in reviewing the general fund and health sustainability fund proposals.

Board members noted the city inaugurated a new mayor on the day of the meeting and that the new administration had announced further hiring restrictions at an earlier department‑head meeting; Abby Yant, SFHSS executive director, said the new mayor indicated an immediate hiring freeze and that departments will receive additional direction from the mayor’s office. Hussain said SFHSS will submit budget proposals in accordance with the mayor’s guidance and will look to maximize operational efficiencies and limit nonessential hiring where required.

No action was taken; staff said the board will receive a proposed general fund budget and related materials in forthcoming meetings and will have an opportunity to comment at the Finance Committee and at the regular board meeting in March.
