Stephanie Einfield of Northwest Colorado Health presented the county commissioners with a three‑year campaign to sustain home health and hospice services that Northwest Colorado Health provides in Moffat County.
Einfield said the organization provides in‑home services in the county and that 45% of its home‑health and hospice patients live in Moffat County. She said staff perform roughly 10,000 in‑home visits and drive about 100,000 miles annually for those services. Einfield described the reimbursement environment for home care as difficult and said the organization budgets an approximate $900,000 annual loss for home services without additional community support.
Northwest Colorado Health seeks to raise $3 million over three years to sustain home health and hospice and to create a glide path while pursuing advocacy and partnerships to improve payment models. Einfield said the campaign’s three‑year target would produce an estimated 13,500 additional in‑home visits for Moffat County residents over that period. She noted some partners—including Memorial Regional Health and UCHealth—have provided or pledged support; Memorial Regional Health contributed $100,000 last year and is committed to support next year, Einfield said.
Einfield also reported a private donor had offered a $100,000 matching grant to kick off the campaign and that about $50,000 had been raised toward that match. She asked the board to consider county partnership or funding as part of broader community support but did not present a formal funding request or ask for an immediate vote.
Commissioners thanked Einfield for the presentation and requested updates as fundraising plans develop. No formal county appropriation or action was taken at the Oct. 28 meeting.
