ISBE cyclical monitoring finds procedural IEP issues; district submits corrective action plan and starts internal audits

Summary
The district was part of ISBE’s first cyclical monitoring cohort and reviewed 25 randomly selected IEPs; most problems were procedural (missing excusal forms, mismatched notices), two files required correction and families were notified. Administration has instituted monthly internal fidelity checks and expects follow‑up with ISBE through March 2026.
Huntley Community School District 158’s assistant superintendent for special services updated the board on the district’s ISBE cyclical monitoring, part of a statewide review of special‑education compliance.
ISBE randomly selected 25 IEPs for review. The district found mostly procedural deficiencies—examples included missing excusal forms where invited IEP team members were excused and mismatches between meeting notices and sign‑in rosters. The district submitted a corrective action plan, made required corrections in the two cases that required changes and notified the families involved. Administrators said they obtained family sign‑offs where corrections were made.
To improve ongoing compliance, the district has launched monthly internal audits and fidelity checks by specialist coordinators and is providing professional development for case managers and administrators. The assistant superintendent said the district expects to complete all required follow‑up with ISBE through March 2026 and will continue oversight of off‑campus placements where the district is still responsible for IEP compliance.
Board members asked how IEPs were selected (ISBE randomly selected 25 by SIS ID across grades and placements) and whether families were notified when corrections were required; administration said yes, those families were informed and provided copies of corrected documents.
