SFPUC staff briefed the commission on fluoride following a constituent—oncern and a recent study published in the Journal of the American Medical Association (JAMA). The study linked prenatal fluoride exposure in some cohorts to small differences in child IQ, but staff and public-health partners described the results as difficult to interpret and not a basis for immediate policy change.
Steve Ritchie, assistant general manager for water, said California law requires fluoridation of public water systems with more than 10,000 connections and noted SFPUC dds fluoride at an optimal level of 0.7 milligrams per liter (parts per million), below a public-health goal cited in the meeting of 1 mg/L. Ritchie described fluoridation as an effective and equity-oriented public-health measure to reduce dental caries, particularly in disadvantaged communities with less access to routine dental care.
Ritchie said the department had contacted the Department of Public Health staff and noted the American Academy of Pediatrics found the JAMA study results difficult to interpret because the observed IQ differences were small and varied by sex. He said SFPUC currently includes guidance in its annual Consumer Confidence Report (CCR) about preparing infant formula with low-fluoride bottled water and that staff may revise the CCR if scholarly and regulatory consensus evolves.
During public comment, speakers asked the commission to seek independent expert testimony (e.g., academic or UCSF specialists) to address outstanding questions about exposure pathways, groundwater blending, and monitoring.
Next steps: staff will monitor the peer-reviewed literature and regulatory guidance and said they take new research seriously. The commission did not adopt any new policy in the meeting.
