The San Francisco Health Commission voted unanimously to adopt a resolution supporting the 2016 Community Health Needs Assessment (CHNA), a report prepared in collaboration with the San Francisco Health Improvement Partnership.
"The community health needs assessment was completed in collaboration with the San Francisco health improvement partnership," Michelle Kern, an epidemiologist with the Population Health Division, said while summarizing revisions to the report. Kern described changes made since the April presentation, including renaming the document to align with nonprofit hospital needs-assessment requirements, correcting typographical and labeling errors, and adding maps and cross‑referencing tables between neighborhood designations and ZIP codes.
Kern noted limitations in data availability for some populations, including transgender residents, and said the report now includes an appendix describing those limitations and the department's plans to improve data. Commissioners asked how the assessment would inform the subsequent community health improvement plan; staff said they expect the improvement plan to be completed by mid‑summer and that the assessment will feed strategic planning and hospital community‑benefit plans.
The commission moved and unanimously adopted the resolution supporting the assessment. Commissioners requested follow‑up presentations on the improvement plan and clearer performance metrics tying the assessment to departmental strategy.
