The Health Service Board received financial updates on June 13 showing ongoing pressure on the benefit trust and city budgeting choices that affect SFHSS programs. Chief Financial Officer Iftikhar Hussain told commissioners that the trust balance is projected to decline because of rate stabilizations, rate buy‑downs and prior settlement dollars applied to buy down rates.
Hussain explained mayoral changes to the previously approved general fund administrative budget: the mayor removed $600,000 in general‑fund wellness program funding and staff proposed absorbing those activities within the Health Sustainability Fund so programming could continue. The board later approved the revised Health Sustainability Fund budget so those wellness positions and activities continue; staff presented an updated five‑year forecast showing the sustainability fund remains solvent through FY2027 with the change.
Members of the public asked clarifying questions about the trust balance and how a reported $6,000,000 projected decline in the trust occurred; staff clarified the projection reflects the combined effect of 2024 rate stabilization, use of Sutter settlement funds to reduce rates, and favorable claims experience in certain lines, and that trust funds are restricted to benefits. The board approved the Health Sustainability Fund budget unanimously and asked staff to continue monthly financial reporting and to include outreach about member impacts where relevant.
