Blue Shield moves infertility meds to pharmacy benefit; board hears member testimony of major out‑of‑pocket costs

Blue Shield of California told the San Francisco Health Service Board that it has restructured its infertility authorization and drug‑coverage process to reduce barriers to care and improve member access.
Sean Levering of Blue Shield said the carrier removed prior authorization and a diagnostic requirement for infertility services effective Nov.1, 2020, retrained providers, created a dedicated internal subject‑matter expert, reprocessed denied claims back to Jan.1, 2018, and moved infertility medications from the medical benefit into the pharmacy benefit with CVS Specialty to enable home delivery and consistent pricing. Levering said the transition began with a soft migration on Nov.1 and that medical‑benefit coverage of certain drugs will be retired on Jan.1, 2021.
During public comment Erica Maybaum, a member, described repeated denials of infertility care and said the policy changes will materially reduce costs for affected members. Maybaum said she spent $6,500 out of pocket on medications last year and welcomed the coverage shift as both a quality and equity improvement that could make family building possible for members who otherwise could not afford it.
Board members thanked Blue Shield and staff for working with members and for the administrative changes. Commissioners asked how far back claims were reprocessed (Blue Shield said to 01/01/2018) and requested that public-facing materials (the Blue Shield microsite) be updated to reflect the policy changes.
The discussion was informational; no procurement or contract change vote was taken during the meeting.
