Mister Phillips, the district auditor for safety operations, told the board the district completed 10 audits in the past three months and “we did have, 4 failures this last time.” He identified specific failures at Meadow Creek, Trinity High School (a portable building door), Euless (a kitchen exterior door) and Bedford Junior High, where Phillips said the access‑control system at the front door was down while the auditor was on site. “For the Bedford Junior High audit, I am appealing that one,” Phillips said, calling out that a vestibule prevented unescorted movement onto campus even though the front-door access control was not operating.
Phillips said some failures reflected construction activity or transient staffing at gates that allowed auditors to reach doors they would not typically access. He said the district is pursuing maintenance, retraining and repairs, and that two audits remain this month with three more scheduled next month.
A board member asked whether the district will retrain staff or repair hardware. Phillips said the response includes communicating with campus staff, engaging maintenance to check doors, and ongoing follow‑up: “It’s really just something that I’ve had to push through, making sure that we’re actively checking on that.”
