Ken described programs funded by the grant: Fast Track Home, which provides short‑term services and durable medical equipment to help older adults transition home from hospitals or nursing facilities and avoid readmission; utility assistance; emergency housing help; home modifications such as handrails; and AddInAide, a caregiver‑matching app designed to reduce waiting lists for home care by expanding a flexible local workforce.
Commissioners praised the program’s efficient use of funds and its role in keeping seniors safely at home. The director said he will work with Ken over the next month to propose ways to align levy funds for similar services and to bring forward any recommended contractual or program adjustments for the board’s consideration.
