Contra Costa Health Plan (CCHP) and Brentwood city staff presented a partnership to establish a CalAIM center at the Brentwood Senior Activity Center. City Recreation Supervisor Lindsay Pinnell and CCHP quality manager Lisonbee detailed services and funding in a Dec. 9 presentation.
CCHP will award $80,000 over two years (two annual installments of $40,000) to support supplies and programming at the senior center, staff said. The CalAIM model offers enhanced care management, medically tailored meals, housing navigation and asthma remediation among other community‑based services for Medi‑Cal members; CCHP will help tailor offerings to the senior center’s needs and coordinate reporting and semiannual outcomes tracking.
Council members welcomed the opportunity. Staff said the funding could offset cuts from county programs and cover transportation and meal costs that support seniors’ participation in activities.
Next steps: staff will finalize agreement terms with the County, set a kickoff meeting and create an implementation schedule. Semiannual reports will outline activities, expenditures and community outcomes.
Ending: If executed, the Brentwood Senior Activity Center will be among the first municipal CalAIM centers in Contra Costa County, according to staff.
