WASECA, Minn. — Waseca County public health reported recent managed-care audit results and outlined corrective action plans aimed at improving documentation and CMS compliance.
A public-health presenter summarized the Blue Plus audit: of 16 elements reviewed, one required a documented improvement plan and several findings related to documentation and assessment workflows were identified. "Blue Plus came out of 16 elements, only 1 required a documented improvement plan," staff said, adding that the corrective action plan includes internal chart audits to catch misses earlier.
South Country Health’s audit covered a larger sample and identified that roughly 12% of elements did not meet standards in two categories; public health staff said similar corrective actions will be used, including checklists and targeted chart reviews.
Staff emphasized the audits related to compliance with managed-care contracts and CMS regulations rather than to the quality of client services; they said the county believes residents are receiving services but documentation must improve. Sam Langer was identified as the senior services lead on the audits.
What happens next: Public health will implement the corrective action plans, perform chart audits and return to the board if additional resources or policy changes are required.
