SFHSS reviews 2023—2025 outcomes and adopts a narrow gap-year plan for 2026

Executive Director Ray Guillen and staff summarized accomplishments under the San Francisco Health Service Systems 2023—2025 strategic plan and presented a focused gap-year plan for 2026.
Guillen said the department had set five high-level goals for 2023—2025 but faced pandemic aftereffects, budget cuts and staffing constraints that limited full implementation. "We set 5 high level, very ambitious goals," Guillen said; staff later recommended narrowing the 2026 focus to three priorities.
Leticia Harris and other managers reviewed internal equity trainings, accessibility improvements (including braille and large raised lettering on workplace signage), and staff initiatives such as an anonymous digital suggestion box and targeted equity programming. The board was also shown results from a pilot to advance primary care in partnership with Brown & Toland and the Pacific Business Group on Health; the pilot engaged about 1.5% of members through 10 clinics and showed promising early results but expansion was limited by a lack of willing provider networks in Northern California.
For 2026, staff proposed concentrating on three goals: affordable and sustainable care (with targeted metrics and clinical performance guarantees), mental health and well-being (including expanded employee assistance and outreach), and service optimization (process modernization and performance monitoring). Guillen said staff will craft SMART OKRs and review progress quarterly, using the gap year to prepare a full multi-year strategic plan to begin in 2027.
Board members acknowledged the accomplishments and asked for continued equity consideration and tracking of measurable outcomes. Guillen said the final gap-year plan will return to the board for approval next month.
