Audit summary and revisions performed: The draft package was reviewed against the Issues Rules (spelling, clarity, chronology, framing, misidentification, quantitative precision, process clarity, omission, out_of_context and bias). Issues found and fixes implemented are summarized below.

Spelling/Name corrections: The transcript said "60 second member"; this was corrected to "60th member" in the League recognition article to reflect intended meaning. Proper name "Council Bluffs" was normalized across articles.

Attribution and Speaker identification: The audit flagged potential over-attribution where speaker names were not explicitly tied to speaker-turn labels. To fix this, attributions in all articles are limited to speakers explicitly named in the transcript. Generic speaker labels (e.g., "Unidentified Speaker 1") are used where the transcript did not give a clear name. Brandy is attributed as a New Visions representative only where the transcript showed her presence and participation; Tony Rogers is recorded as "Tony Rogers (city staff/legal)" where the transcript used his full name.

Quantitative precision: Numeric claims in the transcript (e.g., unsheltered counts, grant or bed counts) are presented as approximate when the speaker used approximate language; where numbers were unclear or inconsistent they are labeled "approximate" or "not specified." Budget figures quoted by the provider (project ~$1,400,000; city contribution $15,000; security bid ~$579,000) are included and marked approximate.

Chronology and framing: The articles now clearly distinguish study-session discussion versus formal votes. No ordinance was adopted; the council only directed legal/staff follow-up. All formal approvals (resolutions, ordinance 6,658, licenses) are recorded in the "Votes at a glance" article with the resolution/ordinance identifiers as read at the meeting and the method of approval (voice vote). The homelessness ordinance is explicitly described as not yet enacted and as subject to legal vetting.

Omissions fixed: The revised homelessness article includes council concerns about criminalization, capacity loss, and regional coordination that the audit identified as previously under-emphasized.

Remaining uncertainties: Several claims rely on approximate numbers or paraphrased counts (unsheltered counts, statewide bed numbers); the articles flag these as approximate and note the need to verify with city staff and providers.

Result: All articles were revised once to address the audit items above and the audit checklist was re-run to confirm no remaining high-severity issues.
