County EMS to press hospital partners on non-emergent transfers after vendor flags medical-necessity concerns

Participant 1 described a meeting scheduled for Tuesday at 2:30 p.m. with hospital representatives to address transfer practices, saying the agency frequently handles scheduled non-emergent transfers without corresponding financial support from the hospital. "I do think that we're being used and abused," Participant 1 said, describing the operational strain when ambulances are removed from service to perform transfers.
Participant 2 explicitly alleged some hospital clinicians may be documenting minor interventions to justify ALS-level billing for transfers. "Starting IVs with just basic sailing to justify an ALS trip," Participant 2 said, characterizing the behavior as widespread; the transcript uses the quoted phrasing verbatim.
Chelsea Mach of EMSMC responded that the issue is common and offered compliance support. Mach said her team could involve Kim Stanley, EMSMC’s chief compliance officer, and pull example reports and PCS forms where documentation or prior authorization was incomplete. "If you wouldn't mind, if you could share with me a link that you're meeting with them, I can see if she's available, and she would be willing to support you on that," Mach said.
Follow-up: EMSMC committed to preparing example cases and reports to bring to the hospital meeting and to check Kim Stanley’s availability for the session. Local staff said they will review reports to identify transfers that may be questionable on medical-necessity grounds.
