Board amends charges, accepts memoranda of decision; two licenses revoked

The nursing board on Feb. 4 approved an amendment to a statement of charges and accepted memoranda of decision that produced disciplinary outcomes.
Julianne Kataya, a Department of Public Health attorney, told the board she had filed a motion to amend a statement of charges to correct an error that used a patient initial rather than a number. "I am Julianne Kataya, staff attorney for the Department of Public Health," Kataya said, and explained the amendment was limited to correcting that error. The respondent, Ralph Hirschfeld, APRN, said he had no objection. Mary Dietman moved to approve the amendment and the board recorded a roll-call 'Aye' that carried the motion.
The board then considered memoranda of decision written by hearing officers. Lisa Freeman moved to accept the MOD for Yesenia Cruz (RN, petition 2025-932); Elizabeth Rivera Rodriguez seconded and the board recorded unanimous 'Aye' votes among present members. The chair confirmed the respondent understood the revocation outcome.
A proposed MOD for Abiola (RN, petition 2025-878) was presented as a revocation after a full hearing. Mary Dietman moved to accept that proposed MOD; Cindy Arpin seconded. After roll-call, the board approved the order and the chair informed the respondent that his license had been revoked.
All formal votes were recorded on the public record with roll calls taken by the chair. The actions followed standard procedure for handling hearing officer decisions and statutorily required due-process steps when summary suspensions or revocations are involved.
