Fargo Cass Public Health board backs AAP/AAFP childhood immunization schedule

The Fargo Cass Public Health Board of Health voted unanimously Jan. 9 to endorse continuing use of American Academy of Pediatrics and American Academy of Family Physicians childhood immunization schedules in its local policies and standing orders, citing concerns about a recent CDC/HHS posting issued before advisory committee review.

Health Officer Dr. Jesse Lindemann told the board he was seeking the board’s support because the federal posting came before the Advisory Committee on Immunization Practices (ACIP) could meet and provide guidance. "We would like to continue with evidence-based practices and continue with the AAP and AAFP vaccination schedules as we have previously," Lindemann said, asking members whether they supported that direction.

Jen Fall, director of Fargo Cass Public Health, explained the potential operational and financial risks if the federal schedule is adopted without ACIP alignment. She said the county’s concern centers on changes that could affect dosing recommendations; for example, staff warned that under certain interpretations the HPV vaccine could move to a single-dose recommendation and that any change in vaccine purchasing or reimbursement could have local financial implications. "One vaccine that we are concerned about is the HPV vaccination, which is scheduled for a one-time dose now with the new CDC recommendations. Former schedule is the multiple doses. And each one of those shots is roughly $300," Fall said while noting potential exposure if VFC reimbursements shift.

Board members discussed whether the request should be a formal vote. After discussion, the board held a roll-call vote on a motion supporting Fargo Cass Public Health’s continued use of AAP/AAFP guidance in its policies and standing orders. The motion passed on a unanimous recorded vote.

Why it matters: local public-health programs set standing orders and clinic processes that affect how vaccines are scheduled, purchased and billed. The board and staff said they sought clarity from ACIP — which is scheduled to meet in February — so local operations would not be forced to change twice in rapid succession. Staff emphasized that continued adherence to AAP/AAFP guidance is meant to reduce confusion for providers and families while the national advisory process concludes.

What’s next: Fargo Cass Public Health will continue current clinic practices under AAP/AAFP guidance and monitor ACIP’s February deliberations. If federal recommendations change and VFC or other reimbursement policies shift, the board will revisit the policy and any necessary budget implications.
