Hearing on Cornerhouse Care discharge notice spotlights $4,084 monthly charge and resident’s housing, health struggles

A hearing officer heard testimony from Cornerhouse Care administrator Carrie Cardwell and resident Daniel Michael Neslak in an administrative proceeding over a discharge notice tied to alleged nonpayment.

Cardwell, sworn as the facility's administrator, told the panel the resident had been at the facility since "11/05/2025" and that "there's non nonpayment to the facility." She said the monthly charge for the room was "$4,084 a month." Cardwell said the facility applied for state assistance on the resident’s behalf but that the application was ultimately denied: "I was thinking that it would be paid in full, and then it was denied," she said; when asked what she meant by "saga," she explained, "Like, the state pays in full for him to live here." The administrator said the resident signed a lease at move‑in and agreed to file a copy of that lease with the hearing office as "late file number 1" by the following Wednesday.
