UDS 2025: HRSA adds new reporting fields for SUD, MOUD, tobacco pharmacotherapies and removes some SOGI lines

Summary
HRSA's 2025 UDS updates include removal of the patient sexual‑orientation/gender‑identity section in Table 3b, new Table 6a lines for tobacco cessation pharmacotherapies and MOUD, new Alzheimer’s screening fields, and new substance‑use disorder quality measures in Table 6b; the webinar highlighted validation logic that will block saves until mutually exclusive answers are corrected.
HRSA trainers used a UDS webinar to highlight several substantive 2025 reporting changes that health centers must map and implement in their internal systems before submitting the 2025 UDS report.
Rashmita Bean, who led the demonstrations, said that Table 3b no longer includes the patient-by-sexual-orientation/gender-identity section and that several previously reported lines in Table 9e (including 1l, 1m, 1n, 1p, and 3b) have been removed for this year. She advised centers to consult the 2025 UDS manual for a comprehensive list of label and code changes.
On service‑and‑clinical reporting, Bean noted three new required lines in Table 6a: “26c2, tobacco use cessation pharmacotherapies,” “26c3, medications for opioid use disorder,” and a new Alzheimer’s/dementia screening line (26f). Each of these new lines requires both number-of-visits and number-of-patients counts.
Bean also introduced new quality measures in Table 6b for substance use disorder (section N, measures 23a and 23b), which require reporting for patients age 13 and older on counts such as new SUD episodes, records reviewed, and patients who received SUD treatment.
The presenters demonstrated new validation behavior across multiple questions: selecting a negation option such as “none of the above” will be treated as mutually exclusive and will prevent saving until the contradictory selections are corrected. They emphasized health centers must adjust mappings, renumbered footnotes, and back‑end calculations to match 2025 changes.
For implementation details and full lists of removed or changed fields, HRSA directed users to the 2025 UDS manual and the BPHC UDS training and technical assistance resources.
