WMSC: red-signal overruns rising in 2024; oversight increases and pilot training highlighted

WMSC investigators told commissioners that red-signal overrun events have increased in 2024 and that the agency is stepping up independent reviews of MetroRail data and oversight work.
Mister Quigley said the commission’s data review identified 12 red-signal overruns so far this year, surpassing nine reported in 2023. He described recent cases—Downtown Largo (Aug. 30) and others from January and February 2024—where operators or maintenance crews moved past red signals without positive confirmation because of radio failures, unclear permissions, or misinterpretation of alphanumeric location codes.
“Causes and contributing factors related to these events include noncompliance with written operational rules and procedures, inadequate communication practices, poor radio transmission quality, and a lack of training and supervisory oversight,” Quigley said.
Commissioners pressed on remedies. Vice Chair Lobby, Commissioners Ross and Rouse emphasized territory familiarization (knowing signal locations), expanded simulator training to create realistic stress scenarios, and clearer radio/controller protocols (distinguishing the roles of button controllers who set routes and radio controllers who speak to crews).
Quigley and other WMSC staff noted pilot programs are underway—such as a territory-familiarization pilot for operators—and said WMSC will follow outcomes closely. Commissioners urged that pilots, if effective, be expanded quickly and that outcomes be reinforced with on-the-ground efficiency testing and supervisory observations rather than relying solely on retraining after incidents.
WMSC also identified communication-system problems in rail yards and shops and has an open CAP (C0100) addressing yard/shop radio issues; staff said that cap does not currently extend to mainline radio performance.
What happens next: WMSC will continue independent data reviews, present final investigation reports as completed, monitor pilot programs, and consider program-level CAPs or broader corrective measures if systemic causes persist.
