The Spencer County Fiscal Court approved a $9,840.67 payment arising from the state’s final reconciliation of the 2021 Ambulance Provider Assessment Program (APAP), county staff said during the Dec. 1 meeting.
EMS Director Robert Klinglesmith presented the Department for Medicaid Services' reconciliation documents and summary letters explaining the payer-level calculations and the required transfers between payors. The packet included a table of line-item calculations; county materials described a net payment obligation captured in the meeting materials and motion records.
On a motion by Magistrate (Esq.) Cotton, seconded by Esq. Stump, the court approved payment of $9,840.67 connected to the 2021 APAP reconciliation; the motion passed by voice vote with all members present recorded as "Aye."
Klinglesmith also presented an informational notice about the Spencer County EMS medical director contract. University of Louisville Physicians notified county EMS leadership that UofL was increasing administrative taxes and overhead by 10% and adding a 4% Dean’s fee — a total 14% increase. The notice said, "This would not increase the hours for the medical director and would remain the monthly commitment of 7 hours," according to the included UofL email from Aaron R. Kuzel, D.O.
Why it matters: The APAP reconciliation is a state-directed payment program; reconciling interim payments to actual utilization can produce amounts due to or from providers. The approved payment finalizes the county's recorded obligation for the 2021 reconciliation. The UofL contract increase will raise annual EMS medical-director costs by $2,352 (from $16,800 to $19,152) absent changes to hours.
