Clarissa Rogers, director of the Clay County Health Department, presented the department's updated records-retention and disposition schedule and requested board approval of the signature page required for state filing.
Rogers reported the Community Health Improvement Plan (CHIP), which supports the Community Health Assessment (CHA), was submitted and officially approved by the state in late October. She said the department's laboratory completed a CLIA audit in October with zero compliance issues, demonstrating adherence to quality standards. Rogers also told the board the department is preparing for a hybrid accreditation site visit scheduled for Jan. 8, 2026, which will include dashboard-based data review and an on-site evaluation.
Commissioner Scott Penland moved to approve the record-retention schedule; Commissioner Clay Logan seconded the motion, which passed unanimously (4-0). The minutes indicate the updated signature page will be sent to the state for filing but do not include the signature text.
