Vermont startup Aprexa pitches medication-therapy management for Medicaid

Rob Squire, founder of Vermont-based Aprexa, introduced his company to Senate Health & Welfare on March 17 and described a medication-therapy management (MTM) service the company provides to Medicare and commercial plans elsewhere. Squire said MTM and medication-optimization services can improve health outcomes and generate cost savings by reviewing polypharmacy, preventing adverse interactions and identifying lower-cost therapeutic alternatives.
Squire told the committee Aprexa has provided MTM services for Medicaid and Medicare plans in other states and suggested the company could help Vermont Medicaid address rising pharmacy spend, which he said grew 54% in the state from 2020 to 2023. He framed the proposal as both a health-improvement and economic-development opportunity that could bring jobs to Vermont.
Committee members asked how MTM would integrate with existing state efforts such as the Blueprint for Health and whether services would be delivered at the payer or provider level; Squire said MTM could be implemented in either model and offered to meet with Medicaid to explore contracting options. No procurement or contract decisions were made during the session.
