County outlines Pathways initiative, new housing support and transitional housing plan

Ross said the county is helping to develop a specialized case‑management unit and a new housing support program that leverages a Medicaid reimbursement benefit established earlier in the year for case management tied to housing instability. He said the county is planning transitional housing at the Hillview campus and aims to hire peer support specialists who will focus on helping people navigate housing searches and connect to services whether or not they enroll in CCS.
Vendors asked how centralized coordination would affect clients and whether shared lists might inadvertently block access to multiple supports; county staff suggested vendors engage with the weekly case‑conferencing meetings to coordinate supports and reduce duplication.
