SoHum Housing Opportunities approved for Case Conference Committee; qualification agreement to be circulated

Committee members noted the Case Conference Committee’s membership historically followed organizations listed on the HMIS Release of Information (ROI), a list that became functionally dormant in 2022 after the county’s Notice of Privacy Practices was amended to permit data sharing with the statewide Homeless Data Integration System (HDIS) and Partnership HealthPlan. Robert Ward (DHHS) drafted language for a written qualification agreement and was directed to circulate it for review at the next Executive Committee meeting.
Why it matters: the Case Conference Committee is the forum where service providers and leaders coordinate housing placement decisions driven by the Coordinated Entry System (CES). Formalizing membership procedures aims to preserve continuity of care while establishing transparent, consistent access for organizations that need CES coordination.
Details: The Executive Committee approved SoHum Housing Opportunities because the organization is listed on the ROI and is operating an Encampment Resolution Funding (ERF) project. The Committee agreed to delay admitting other organizations, directing short-term post-hospitalization and medical-respite providers to work through existing CES access points for continuity of care rather than immediate Case Conference membership. Robert Ward (Department of Health and Human Services) will circulate the draft qualification language and the item will be agendized for formal approval at the next EC meeting.
