CSB: Hurricane Laura damage at Biolab Lake Charles triggered chlorine release and fire; report cites five safety failures

The U.S. Chemical Safety Board said Hurricane Laura’s Aug. 27, 2020 landfall damaged the Biolab Lake Charles chemical facility, allowing rainwater to contact a TCCA-based formulation and triggering a reaction that produced toxic chlorine gas and a fire that destroyed a production building.
The CSB investigation found no reported injuries but concluded the incident exposed multiple safety and regulatory failures that allowed more than 1 million pounds of reactive product to remain vulnerable during the storm. “Companies and regulators must take steps to ensure that chemical facilities are protected against damage from natural hazards as extreme weather is becoming more common,” the video narration states.
Why it matters: The CSB says the incident demonstrates how extreme weather can convert stored chemical products into acute public-safety hazards when facilities are not designed, assessed and maintained for current risk levels. The board recommended that Louisiana require operators at risk from extreme weather to evaluate and safeguard facilities and urged the EPA and OSHA to close regulatory gaps for reactive chemicals.
What happened: Forecasters on Aug. 24 warned Tropical Storm Laura would become a hurricane; Biolab removed roughly 825,000 pounds of product from the site but scheduled shipments to remove the remainder never arrived, leaving over 1 million pounds onsite. Management placed remaining product on triple-stacked pallets and evacuated employees at 1:00 a.m. on Aug. 27. Laura made landfall as a Category 4 hurricane; the CSB video reports maximum sustained winds of about 150 mph and heavy rain that ripped roof sections from plant buildings.
Rainwater contacted product stored in a production building that held an estimated 70,000–100,000 pounds of the TCCA-based formulation and in a damaged warehouse that the video says contained over 1 million pounds. The material reacted, generated heat, decomposed and released toxic chlorine gas; the ensuing fire produced a large plume that prompted shelter-in-place orders for nearby residents and closure of Interstate 10. Emergency responders worked to control the release; the facility was considered sufficiently controlled about 28 hours later.
CSB findings: The board identified five safety issues it says contributed to the incident: 1) inadequate extreme-weather preparation and failure to implement updated CCPS guidance; 2) incomplete process hazard analyses (PHAs) implementation and poor tracking of PHA action items — the video says a 2010 PHA had identified warehouse decomposition risk but there was no documentation the recommended roof evaluation occurred; 3) emergency-preparedness and response failures, including an inoperative backup generator, personnel difficulty activating a rented generator and insufficient fire-pump capacity until a third-party contractor arrived more than five hours after the first 911 call; 4) lack of extra protections required by applicable National Fire Protection Association codes (NFPA 101 and NFPA 400), such as automatic sprinkler or other systems for high-hazard occupancies; and 5) a regulatory coverage gap for reactive chemicals — the video notes TCCA is not listed under OSHA’s Process Safety Management standard or the EPA’s Risk Management Program rule.
Recommendations and next steps: The CSB recommended Louisiana require hazard evaluations and safeguards for facilities vulnerable to extreme weather and reiterated prior recommendations that OSHA and EPA amend regulations to address catastrophic reactive hazards. The video also references a GAO report urging EPA guidance to incorporate climate-change risks into inspections and risk-management planning.
