Morrison County signs Purchase of Service with Habitat for Humanity for 2026–27 housing aid

Morrison County Health and Human Services has executed a Purchase of Service Agreement with Habitat for Humanity of Morrison County to administer Statewide Affordable Housing Aid for residents of Morrison County, the contract shows.

A May 8, 2026 cover letter to Kathy Lange, executive director of Habitat for Humanity of Morrison County, transmits the contract and lists required attachments, including an affirmative action form, excluded-provider checks (OIG), insurance and license documentation, and an affirmative-action certification requirement when applicable.

The agreement term runs May 1, 2026 through Dec. 30, 2027. Attachment A specifies program eligibility and funding allocations: $15,000 for "Aging in Place" repairs, $20,000 for closing an "Affordability Gap" for construction costs, and $14,009 for a Habitat & School District Partnership involving Little Falls High School — a sum that Attachment A caps at $49,009. The county must approve any allocation adjustment exceeding 5% and must monitor reporting on families and persons served.

Payment provisions require the provider to receive the allocated dollar amount in a lump sum upon execution; the provider then submits monthly receipts documenting dollars spent, counts of distinct families/youth served, and total persons served. Any remaining funds at contract end must be returned to Morrison County Health and Human Services.

Contract provisions include standard compliance clauses: adherence to state statutes listed in the agreement (including Minn. Stat. §256.0112 and §477A.36 where cited), provider recordkeeping subject to county and state audit, worker’s compensation compliance, non-discrimination under Minn. Stat. Ch. 363A, insurance requirements (general liability and professional liability minimums), and a debarment/exclusion certification requirement that directs providers to the OIG exclusions database.

The contract contains a Business Associate Addendum (BAA) addressing HIPAA obligations for protected health information, breach notification, safeguards for electronic PHI, and special instructions for substance use disorder records governed by 42 C.F.R. Part 2. The BAA requires the provider to report breaches promptly and indemnify the county for unauthorized disclosures.

The agreement is signed by Kathy Lange, executive director of Habitat for Humanity of Morrison County, and Nathan Bertram, director of Morrison County Health and Human Services.
