Livingston County nursing home reports substantial compliance, disputes $953,550 in Medicaid overpayments

The committee approved a payor contract with UnitedHealthcare’s Veterans Affairs Community Care Network, set to commence June 1, 2026, and automatically renew annually unless terminated. The contract was presented for approval and carried by committee motion.
Spencer summarized recent regulatory and financial developments. The facility’s overall health inspection star rating dropped to two stars after a recertification survey; quality measures and staffing ratings remained at four stars. Financially, the CNR reported cash reserves of $1,229,876.21 as of April 29, 2026.
Spencer also reviewed the results of an audit by Performant (OMIG agent) covering March 1, 2020, through Dec. 31, 2024. The audit identified 420 overpayment claims totaling $955,461.21; per the report, the facility is disputing $953,550.02 (396 claims) and agreed to $1,911.19 (24 claims). An appeal was submitted on Dec. 8, 2025.
Facility staff outlined ongoing operational steps: continued recruitment for a Deputy Director of Long Term Care, planned CNAT training classes beginning June 14, and negotiations to adopt OnShift scheduling software to better monitor staffing and overtime. Beacon will continue billing services during the transition of other vendor contracts.
The committee vote to approve the UnitedHealthcare payor contract was recorded as carried; no roll‑call tally was provided in the minutes. The Performant appeal remains pending and will affect final overpayment liabilities if not resolved.
