USBE expands incident codes, adds ESI fields; LEAs must keep using Qualtrics this year

State incident reporting leads told LEAs that several changes to incident coding and emergency safety intervention (ESI) data are coming and that LEAs should prepare now.
Willie, the incident presenter, said Utrecht will add a credible‑threat flag and that the office expanded infraction codes to give LEAs more specific options (for example, inappropriate language, vandalism, electronic device misuse) and to reduce use of the generic 'other' code. He also said the prior 'assault' code was deprecated and guidance recommends coding many such cases as harassment under federal Title IX processes.
On ESI fields, Willie said last legislative session (Senate Bill 170) added nine ESI fields in total: six were already added to the Utrecht specs and three additional fields (number of physical restraints, number of seclusions, and an ESI duration threshold indicating whether any ESI exceeded 30 minutes) are being included. For the current 2025–26 school year, LEAs are still required to complete the parallel Qualtrics ESI survey even if their SIS has built the fields, because the ESI data committee needs access to the Qualtrics data. For future years, LEAs that have built interoperable fields may not need to complete Qualtrics if the committee accepts that approach.
Willie asked LEAs to avoid defaulting to the 'other/disruption' category when more specific codes apply, and pointed to an appendix with code definitions. Malia added that several ESI changes allow 'select all that apply' (rather than a single value) for specified fields and called out the change to ESI duration reporting so LEAs report the total minutes of ESI per incident.
Practical steps: LEAs should review the updated infraction appendix, confirm whether their SIS can accept the new fields and multi‑select options, continue to submit Qualtrics data for 2025–26, and plan to implement Utrecht/UTRx fields for the next school year.
