CPMT adopts updated CSA parental agreement form to replace 2008 local version

Tazewell County’s Community Policy and Management Team on July 28 reviewed a Model CSA Parental Agreement Form Appendix B and compared it with the county’s existing parental agreement on file since July 1, 2008. Members made changes to the model to reflect Tazewell County’s local language and voted to replace the older form in the local CSA Policy and Procedure Manual effective July 1, 2025.
Lori Gates-Addison moved to approve the updated Appendix B; Michelle Brown seconded. The motion passed unanimously. The update was presented as an "unfinished business" item and recorded as replacing the prior parental agreement in the county’s CSA manual.
Minutes do not include the full text of the revisions; the record indicates the updated Appendix B will become the local parental agreement and be used in county CSA procedures.
