Weld County proclaims Reunification Month and approves human‑services grants and contracts

Tammy Grant, deputy director of the Department of Human Services, told the board that Reunification Month recognizes efforts to help children safely remain at home or reunify with family. She reported 2025 outcomes: 126 child‑welfare cases closed and 48% of those were closed with children safely remaining at home; 94% of intake assessments were transferred or closed with children remaining at home (SEG 391-399, SEG 405-410). The department will mark the month with a public event June 10 to honor families who reunified or safely remained at home (SEG 384-388).
The board approved a standardized professional services agreement for case management agencies conducting Interim Support Level Assessments (ISLA). Grant materials presented an expected reimbursement figure tied to those assessments as $181,662 per completed ISLA assessment (SEG 561-586). Granting a standard contract form is intended to streamline payments and contracting with community providers who deliver case management services.
Commissioners also authorized submittal of a Promoting Safe and Stable Families (PSSF) grant application. Grant documents list a total award amount of $199,998 and a 30% match requirement; county staff said Catholic Charities and A Kid’s Heart will provide partner match and remaining match obligations can be met with existing county funding so no new county dollars are required to apply (SEG 606-616).
Separately, the board approved a contract to name Trestle Programs, Inc. as the fiscal agent for the Collaborative Management Program (CMP), a change intended to transition program oversight and fiscal administration to Trestle (Contract ID 10737). Commissioners praised CMP staff for growing the program (SEG 626-659).
The board approved all items by voice or roll‑call votes without recorded dissents. The department said the combined steps are meant to preserve family stability and ensure payment mechanisms and partner capacity are in place to deliver services and follow‑up supports (SEG 561-625, SEG 626-659).
