County nursing center signs physician services contract amid audits and near-capacity occupancy

The Livingston County Center for Nursing & Rehabilitation presented a professional services contract with Mobile Physician Services, PLLC for medical services beginning July 7, 2026, through July 6, 2029, with two one-year renewal options, at $86,520 per year. The Human Services Committee approved authorization for the chairman to sign the contract.
Director Samontra Spencer reported occupancy of 96.7% (255 residents in-house, seven hospitalized) and cash reserves totaling $593,502.75 as of May 26, 2026. Staffing training and recruitment were discussed; a Certified Nursing Assistant Trainee class was scheduled to begin June 15 with testing in July.
Spencer also briefed the committee on two audits: a CMS Payroll-Based Journal audit covering staffing submissions for Oct. 1–Dec. 31, 2025, and a Performant (OMIG) audit of Medicaid claims for March 1, 2020–Dec. 31, 2024. The Performant audit identified 420 overpayment claims totaling $955,461.21; the facility disputed $953,550.02 (396 claims) and agreed to $1,911.19 (24 claims); an appeal was submitted Dec. 8, 2025. The committee received the financial presentation from Denine Scott, Deputy Director of Finance, and discussed steps for recruitment and responding to audits.
